Definitely internal, rod bearing or wrist pin. Why would you put 91 octane in it?
Sounds like either weak spark, possibly coil or low fuel pressure. It's not going to be the injectors.
Evaporating I imagine, it would be impossible for it to drain into the intake or anywhere else.
